District 6 Supervisor Matt Dorsey warns of Medicaid cuts threatening San Francisco drug‑treatment programs

San Francisco – In a brief interview for Mission Local’s “Meet the Candidates” series, District 6 Supervisor‑candidate Matt Dorsey outlined how looming federal cuts to Medicaid could jeopardize the city’s expanding drug‑treatment and recovery infrastructure.

Medicaid cuts and the city’s budget

Dorsey warned that the Department of Public Health expects to lose between $90 million and $180 million annually beginning July 2027 as tens of thousands of residents potentially lose Medi‑Cal coverage. City officials have projected that the overall federal reduction could cost San Francisco as much as $315 million each year.

Mayor Daniel Lurie’s current budget earmarks $77 million for drug‑treatment and mental‑health services, plus a $34 million reserve intended to cushion future federal shortfalls. Yet the city is already trimming its public‑health system, announcing $40 million in cuts over two years that affect contracts for harm‑reduction services, peer counseling, and workforce development.

Dorsey’s legislative record

Throughout his first term, Dorsey has championed a “Recovery First” approach, citing the Cash Not Drugs ordinance, drug‑free supportive housing, and expanded residential treatment beds as key achievements. He says protecting those investments will be his top priority if re‑elected.

“As the incoming Budget Committee chair, I’m already working with Mayor Lurie, the Department of Public Health and community partners to ensure every Medi‑Cal beneficiary in treatment retains coverage,” Dorsey said. “In some cases that may mean connecting people with jobs so they remain eligible – a wise use of local dollars to keep treatment uninterrupted.”

Local impact and waiting lists

San Francisco’s addiction‑treatment system remains strained. Mission Local reported in April that some residents wait up to four months for a residential bed, despite the city adding more than 600 treatment‑focused beds in recent years. A reduction in Medicaid funding could lengthen those waits and force the city to scale back programs that have become central to its homelessness‑and‑drug‑use response.

Community advocates and families have expressed concern that cuts could “eviscerate” progress made on recovery services, especially for vulnerable populations who rely on Medi‑Cal for continuity of care.

Next steps

Dorsey plans to leverage his upcoming role on the Budget Committee to push for additional local funding streams, including job‑training initiatives that help treatment participants maintain eligibility. He also seeks to preserve the city’s reserve fund and explore partnerships with private providers that can supplement reduced federal dollars.

“We must act now to safeguard the health and safety of our neighborhoods,” Dorsey added. “Our families, our faith communities, and our Constitution‑based right to local self‑determination depend on a strong, uninterrupted recovery system.”
