Stroke Rates Among Young Adults Nearly Double Since 1990s, Study Shows

Researchers at the University of Cincinnati College of Medicine have released a sobering update on stroke risk for younger adults. The study, published in the journal Neurology, tracked first‑ever strokes among people ages 20 to 54 in the greater Cincinnati and northern Kentucky region from 1993‑1994 through 2020. During that 27‑year span, the rate of strokes climbed from 33.9 cases per 100,000 people to 62.2 per 100,000 – an increase of nearly 84%.

Why the rise matters

“Stroke is a very devastating disease at any age. It is particularly devastating when it happens to younger people,” said Dr. David Robinson, senior author of the study and an assistant professor of clinical neurology and rehabilitation medicine. He warned that the long‑term disability from a stroke can make it harder for survivors to work, care for children, and fulfill family responsibilities.

Ischemic strokes – the type caused by a blockage of blood flow to the brain – accounted for most of the increase. The researchers also observed a rise in traditional risk factors: hypertension grew from 47% to 60%, diabetes from 19% to 26%, atrial fibrillation from 1% to 6%, and substance use (largely marijuana) from 5% to 40%.

Not just a local problem

Although the data come from the Cincinnati‑Northern Kentucky corridor, the demographic mix of the region mirrors the broader United States, making the findings relevant nationwide. Dr. Christopher Anderson of Mass General Brigham in Boston noted, “The dramatic rise in rates of stroke are seen here in a population with a racial background, socioeconomic status, and education level that is fairly representative of the US population at large.”

What doctors recommend

Robinson pointed families and young adults toward the American Heart Association’s “Essential 8” checklist: eat a nutritious diet, stay physically active, avoid tobacco, get quality sleep, and manage weight, cholesterol, blood pressure, and blood sugar. He emphasized the importance of having a primary‑care physician who can screen for these factors early.

For young women, particular vigilance is advised. Dr. Michelle Leppert of Tufts Medical Center warned that high blood pressure during pregnancy, post‑partum, and the use of estrogen‑containing birth‑control pills in the presence of migraines with visual changes can increase stroke risk.

Balancing concern with perspective

While the numbers are rising, experts caution against panic. “Strokes are still fairly rare events, so there is no need for young adults to panic,” Leppert said. Instead, she urged regular wellness visits and proactive management of risk factors.

Robinson summed up the public‑health message: “It’s important to start thinking about cardiovascular health when you’re younger, because we probably have the best chance of intervening and reducing bad outcomes if we catch it early.”

Community implications

Local health providers can use these findings to tailor outreach programs, especially in schools, churches, and community centers where families gather. Emphasizing personal responsibility, faith‑based groups, and family support aligns with the values of many in the region and can help curb the upward trend.

As the study authors note, more research is needed to determine why younger adults are experiencing higher stroke rates while older adults have not seen a similar rise. Nonetheless, the clear message is that lifestyle choices, early detection, and consistent medical care remain the most effective tools for protecting the health of our younger generation.
