Philadelphia warns Medicaid recipients to act now as Trump’s new law changes enrollment rules

Philadelphia – City leaders are sounding the alarm for the hundreds of thousands of residents who rely on Medicaid. Under President Trump’s One Big Beautiful Bill Act, major eligibility changes will take effect on Jan. 1, 2027, and officials say swift action is essential to avoid loss of coverage.

Who is affected?

About 645,000 Philadelphians are enrolled in Medicaid. Roughly 180,000 of those individuals fall into the expansion group – adults ages 19 to 64 without children under 14, a qualifying disability, or pregnancy. Those in this group will face new requirements.

Key changes coming Jan. 1, 2027

  • Re‑enrollment will shift from an annual to a semi‑annual schedule.
  • The retroactive coverage window shrinks from three months to just 30 days after an application is filed.
  • Recipients must demonstrate community engagement – 80 hours per month of work, volunteering, training or education.

Crystal Yates‑Gale, Philadelphia’s first deputy managing director, emphasized that many in the expansion group already meet the work‑hour requirement, but they will now need to document it.

Steps residents should take immediately

Yates‑Gale urged residents to download the Pennsylvania Department of Human Services’ Compass app. The app will be the primary channel for receiving notices and uploading proof of work or other required documents. She also advised people to verify that their personal information is up‑to‑date in the state’s system and to read any mail from DHS carefully.

Non‑citizen coverage at risk

By Oct. 1, 2026, certain non‑citizens who lack lawful permanent resident status – including refugees, asylees and some military‑related categories – could lose Medicaid benefits unless they take corrective action.
