Louisville Metro Public Works fires two employees over fatal garbage‑truck incident

Louisville Metro officials announced that two Public Works employees have been terminated for failing to follow established safety policies after a tragic February incident that claimed the life of 35‑year‑old Tyrah Adams. The woman was pulled into a garbage‑truck claw while employees were clearing a vacant lot and alley, and she later died from her injuries.

Investigation findings

An internal Metro investigation concluded the two workers violated departmental safety standards, rules, and ethical guidelines. The Jefferson County Attorney’s Office reviewed the findings and determined there was “not a reasonable likelihood of conviction on any criminal charge,” so no criminal charges were recommended.

Personnel actions and next steps

Executive Director of Public Works Wes Sydnor said the city’s response was swift once the investigation was complete. “Our thoughts continue to be with the family of Tyrah Adams and everyone impacted by this tragic accident,” Sydnor said. He added that the two employees were placed on unpaid leave during the probe and have now been terminated. A third employee involved in the incident retired before the investigation concluded.

To prevent future tragedies, the Metro government has activated an internal policy‑review committee, revised solid‑waste and bulk‑waste procedures, and provided additional training on emergency response. A third‑party consultant has also been hired to conduct a comprehensive review of solid‑waste management policies, with recommendations for further improvements.
