FDA approves daraxonrasib, a new drug that nearly doubles survival for advanced pancreatic cancer

The Food and Drug Administration announced on August 26, 2026 that daraxonrasib has received approval for the treatment of metastatic pancreatic cancer. In a Phase 3 trial of 500 patients who had previously received therapy, the oral medication more than doubled overall survival, raising the median from 6.7 months to 13.2 months.

Why pancreatic cancer has been so lethal

Pancreatic cancer has long carried a grim prognosis because it is rarely detected early and often presents only after the disease has spread. Between 2015 and 2021, roughly 97% of patients diagnosed with metastatic disease died within five years. More than 90% of these tumors are driven by mutations in the KRAS gene, which acts as a cellular switch that, when stuck in the “on” position, forces cancer cells to multiply unchecked.

Targeting the once‑“undruggable” KRAS pathway

For decades scientists believed KRAS could not be directly inhibited because its surface lacks the pockets needed for conventional drugs. Daraxonrasib works around this obstacle by binding to cyclophilin A, a protein that helps fold other proteins. This complex then attaches to the active KRAS protein and blocks its signaling, effectively turning the cancer‑growth switch off.

Trial results and side effects

The trial showed a 60% reduction in the risk of death for patients taking daraxonrasib compared with standard chemotherapy. While more than 86% of participants experienced a skin rash, other common adverse events included mouth sores (stomatitis), diarrhea, nausea and vomiting. Importantly, patients on daraxonrasib were far less likely to discontinue treatment because of severe side effects, and many reported better quality of life and less pain.

Approval of daraxonrasib provides a new, targeted option for those battling advanced pancreatic cancer, a disease that has offered few effective therapies. The drug’s oral administration also simplifies treatment compared with intravenous chemotherapy. Families can look forward to more real‑world data as the medication becomes widely used, helping physicians refine best practices.

Future directions

Researchers anticipate a wave of clinical trials combining daraxonrasib with other agents to prevent resistance and further extend survival. Several next‑generation KRAS inhibitors are also in development, aiming to improve efficacy while reducing toxicity.
