Synthroid and Levothyroxine: What the 2025 Prescription Rankings Reveal About Thyroid Treatment Risks

By OBBM Network Editorial Staff

Derived from an episode of Dr. Ardis Show.

More than a million Americans rely on Synthroid every day, but the medication’s popularity masks a litany of warnings that many patients never see.

Why Levothyroxine Dominates the Prescription Landscape

Levothyroxine, sold under the brand name Synthroid among others, was the third‑most dispensed drug in the United States in 2025, according to the Cleveland Clinic’s prescription data. The hormone replacement therapy is prescribed for hypothyroidism, a condition where the thyroid gland fails to produce sufficient thyroid hormone. By supplying synthetic thyroxine, the drug aims to restore normal metabolism, alleviate fatigue, and prevent weight gain.

Dr. Bryan Ardis notes that the drug’s prevalence stems from the high incidence of hypothyroidism, especially among women over 60, and the simplicity of a once‑daily pill. However, the sheer volume of prescriptions does not guarantee safety.

FDA Warnings and Reported Adverse Events

The FDA’s black‑box warning for levothyroxine stresses that it must not be used for weight loss and that excessive dosing can lead to life‑threatening toxicity. Reported side effects span multiple systems:

  • Cardiovascular: palpitations, tachycardia, hypertension, arrhythmias, myocardial infarction, and even cardiac arrest.
  • Endocrine: altered glucose control, adrenal insufficiency, and hormone imbalances.
  • Neurological: seizures, tremors, insomnia, and headaches.
  • Dermatologic: urticaria, pruritus, angioedema, and skin rashes.
  • Skeletal: increased risk of osteoporosis and premature closure of growth plates in children.

“Levothyroxine increases the risk of death from every cancer in a female that they will develop as a result of the side effects of levothyroxine,” Dr. Ardis warned, citing a separate podcast presentation that examines the drug’s oncologic implications.

Understanding the Types of Hypothyroidism and Treatment Nuances

Hypothyroidism is not a single disease. Dr. Ardis explained four categories:

  1. Primary – the thyroid itself under‑produces hormone, prompting the pituitary to release more TSH.
  2. Secondary – a dysfunctional pituitary fails to signal the thyroid adequately.
  3. Tertiary – the hypothalamus does not secrete enough TRH, disrupting the hormonal cascade.
  4. Subclinical – mildly elevated TSH with normal thyroid hormone levels, often asymptomatic.

These distinctions matter because dosing requirements differ, and overtreatment can push patients into hyperthyroidism, exposing them to the cardiovascular risks listed above.

Practical Guidance for Patients on Synthroid

Patients can mitigate risks by following best‑practice administration guidelines: take the tablet on an empty stomach with plenty of water, wait 30‑60 minutes before eating, and avoid antacids, calcium, iron, or certain cholesterol‑lowering agents within four hours of dosing. Regular monitoring of TSH, free T4, and, when indicated, calcium and bone density tests are essential, especially for older adults.

“Ask your doctor to review the effects of levothyroxine if you’re having cardiovascular issues as a result ever since you started or before,” Dr. Ardis advised, emphasizing the importance of individualized care.

Broader Implications for the Healthcare System

The continued dominance of levothyroxine in prescription rankings raises questions about prescribing habits, pharmaceutical marketing, and the adequacy of patient education. While the drug remains a cornerstone of hypothyroidism management, the breadth of reported adverse events suggests a need for heightened vigilance, alternative therapeutic options, and more robust post‑market surveillance.

As clinicians balance the benefits of normalizing thyroid function against the potential for serious side effects, patients should remain informed partners in their care, demanding clear explanations and regular lab follow‑ups.
