In a development that could change how doctors monitor digestive health, a team of scientists at the University of Maryland has created the first wearable underwear capable of measuring flatulence. The device, a small disc that snaps onto the back of a pair of briefs, uses electrochemical sensors to detect hydrogen gas released during digestion.
Why the technology matters
According to associate professor Brantley Hall, the project’s “ultimate mission is to help people eat high‑fiber diets comfortably with low intestinal gas.” In the United States, about 95% of adults do not consume enough dietary fiber, a nutrient essential for blood‑sugar control, cholesterol regulation, and a healthy gut microbiome. While fiber offers many benefits, it can also increase gas production as gut microbes ferment the fiber, leading to uncomfortable bloating for some individuals.
From data collection to medical insight
The smart underwear is part of the Human Flatus Atlas, a research initiative that recruits volunteers worldwide to wear the device while logging every meal. Although the system is not yet a medical device, researchers believe the continuous, non‑invasive data could eventually help clinicians identify early signs of gastrointestinal problems such as irritable bowel syndrome, celiac disease, or lactose intolerance.
Hall explained that the technology allows participants to go about their daily lives—including sleep—while the device records flatulence events. “There’s this guy; he’s named the King of Farts,” Hall joked, referring to gastroenterologist Michael Levitt, who previously measured gas using invasive rectal tubes. The new wearable eliminates the embarrassment and inconvenience of such methods.
Study results so far
The first study, published in Biosensors and Bioelectronics in December, asked participants to wear the device for about 12 hours a day over a week. Researchers found an average of 32 flatulence events during that period. A larger follow‑up study involving more than 500 volunteers had participants wear the device for at least 22 hours a day over three to 30 days. Results from that study are pending publication.
Dr. Jordan Axelrad, an associate professor at NYU Grossman School of Medicine, noted that continuous, real‑world measurement of microbial metabolic activity could eventually allow doctors to detect gut disturbances before symptoms become severe. However, both Axelrad and Dr. Trisha Pasricha of Harvard Medical School cautioned that the number of flatulence events alone does not diagnose disease. “There isn’t good data showing that the number of farts alone signals something is wrong,” Pasricha said.
Practical guidance for families
For most people, occasional increased gas is normal, especially when increasing fiber intake. Health experts advise that concern is warranted only when flatulence is accompanied by other symptoms such as persistent abdominal pain, diarrhea, constipation, weight loss, blood in the stool, or a sudden lack of bowel movements, which could indicate a more serious condition like a bowel obstruction.
Ultimately, the goal of the Human Flatus Atlas is to build a comprehensive data set that could inform personalized nutrition plans, probiotic therapies, and other microbiome‑targeted treatments. While more research is needed to link specific microbial patterns to disease, the project represents a promising step toward using everyday technology to empower families with better digestive health information.
