Rams coach says Myles Garrett will need knee surgery after 49ers loss

Los Angeles – Head coach Sean McVay announced Monday that defensive end Myles Garrett will undergo knee surgery following the Rams’ season‑opening loss to the San Francisco 49ers in Melbourne, Australia. The 61.5% snap count in the 27‑7 defeat highlighted the severity of the injury, and post‑game conversations with Garrett made it clear a different approach was necessary.

Game exposed the injury

Garrett, who missed most of training camp due to swelling, was unable to record a tackle or sack during the game. McVay explained that the coaching staff had hoped the veteran could work through the issue, but the reality of a live contest forced a reassessment.

“We had been aware of this, thought it was something he would be able to work through,” McVay said. “Obviously, being out there and playing in a real game, you learn like, ‘Hey, maybe that’s not the case. So let’s change course with that.’”

Decision to operate

The Rams placed Garrett on injured reserve, triggering the league‑mandated four‑game minimum before he can return. McVay did not provide a specific timeline beyond that requirement, noting the recovery will depend on the outcome of the procedure.

Garrett was acquired in June from the Cleveland Browns in a blockbuster trade that sent pass rusher Jared Verse and three draft picks to Cleveland. The move was made with the understanding that his workload would be carefully managed, a strategy McVay has employed with veteran players to preserve their effectiveness on game day.

No impact on Aaron Donald’s return

McVay also clarified that Garrett’s absence will not affect the timeline for defensive tackle Aaron Donald, who ended his brief retirement last month. Donald did not travel with the team to Australia as he continues to regain playing shape after two seasons away from the field. The coach said it remains too early to determine whether Donald will suit up for the Rams’ home opener against the New York Giants.

“He’s going to continue to work his process, and we’ll see what that looks like,” McVay said.

Looking ahead

The Rams now face a short‑term challenge on the defensive line while awaiting Garrett’s surgery and recovery. The team’s medical staff will evaluate the procedure’s success and coordinate a rehabilitation plan aimed at returning the All‑Pro defensive end to his pre‑injury form.
