Mobility tests help gauge fall risk but cannot predict lifespan

Doctors and physical‑therapy clinics across the country are using simple mobility assessments—hand‑grip strength, balance checks, and sit‑to‑stand challenges—to spot early signs of frailty in older adults. While these tests can highlight specific weaknesses that raise the chance of a fall, health professionals stress they are not a way to forecast how long someone will live.

What the tests measure

The U.S. Centers for Disease Control and Prevention recommends a 30‑second chair‑stand test for people over 60, counting how many times a person can rise from a seated position. Another popular screen, often seen on social media, asks participants to move from standing to a cross‑legged seat on the floor and back up, scoring points for each movement and deducting points for support used.

Researchers in Brazil published studies in 2020 and 2025 linking higher scores on this sit‑to‑stand test with a lower chance of death over roughly a ten‑year span for middle‑aged adults. However, American clinicians have not widely adopted the test for older patients.

Common clinical tools

Many physicians rely on the “Timed Up and Go” (TUG) test. The patient rises from a chair, walks about three meters, turns, and returns to the seat. The TUG evaluates balance, mobility, and lower‑extremity strength in a single, quick assessment.

Dr. George Hennawi of MedStar Health in Baltimore explains that these tools help pinpoint the underlying cause of a problem—whether it’s knee pain, slowed reflexes, poor balance, or cardiovascular issues that cause dizziness.

Screening, not diagnosis

Experts describe these assessments as screening instruments. They can narrow down potential issues but do not replace a full medical diagnosis. Additional tests—grip strength, walking speed, gait steadiness, and single‑leg balance—are often combined to gauge a person’s “physiological age.”

The Short Physical Performance Battery, a ten‑minute evaluation developed by the National Institute on Aging, bundles gait, balance, and strength measures and has been shown in large trials to predict mortality and disability.

Putting the results in context

Dr. David Lee of UCLA’s Multidisciplinary Falls Clinic advises looking at multiple tests together. Grip strength, for example, can signal many health concerns but may be affected by arthritis, a prior stroke, or simply a lack of conditioning.

Beyond physical measures, comprehensive aging assessments include cognition, hearing, vision, and nutrition. Experts warn against training solely to pass a test; instead, they recommend whole‑body exercise, balanced meals, stress management, adequate sleep, and heart health.

When to seek help

If a mobility screen reveals a poor result, or if an older adult has recently fallen, the next step is to discuss the findings with a health professional. The results may lead to recommendations for assistive devices, physical therapy, medication adjustments, or other interventions.

Emily Nabors, a falls‑prevention specialist with the National Council on Aging, notes that more than a quarter of older adults experience a fall each year, yet many do not report it to their doctors. “If you’ve fallen, you might feel fine, but you might not be fine at all,” she says.
