Louisiana Rural Health Providers Await Allocation of $208 Million Federal Funding

The Louisiana Department of Health announced that the state has been awarded roughly $208 million through the federal Rural Health Transformation Program, a Trump‑administration initiative aimed at strengthening health care in rural communities. The money will be distributed across several categories, including hospital improvements, telehealth upgrades, workforce recruitment, specialty‑care collaborations, alternative payment models, and nutrition initiatives.

Application volume outpaces available funds

More than 500 organizations and providers have submitted applications seeking a portion of the funding. The Department of Health said it will not know how many applications will be denied until October, when the scoring process is complete.

Julie Foster Hagan, executive director of Louisiana’s Office of Rural Health Transformation and Sustainability, explained that applicants may be ineligible, may have failed to provide sufficient information for scoring, or simply ranked lower than competing proposals. “We know that we got a lot more requests than that in the 160 applications that we received,” Hagan told the Louisiana Rural Health Transformation Program Advisory Council.

Capital improvement program capped at 20 percent

One of the most visible components of the program is the capital improvement initiative, which helps rural facilities purchase equipment and make modest renovations. Federal rules limit spending on these projects to 20 percent of the state’s award—about $41.6 million. “This program is capped at 20 percent,” Hagan said. “They say you can’t spend more than 20 percent of your dollars on these types of programs.” Because demand far exceeds the cap, the competition for these funds will be especially fierce.

Telehealth, collaborative models and other priorities

Beyond construction and equipment, the state received 79 applications for telehealth funding, which can cover internet and wiring upgrades as well as technology needed to deliver remote care. Those applications are currently being scored, with award announcements expected by the end of August.

Another 37 applications were submitted for the collaborative provider model, a program designed to help rural hospitals and other providers pool resources to bring specialists into communities that could not support them individually. The state also received 31 applications for alternative payment models, 25 for care conveners and navigation networks, and 37 for the Food is Medicine initiative, which links nutrition services to health care delivery.

Next steps and timeline

Applicants who are not selected will be notified and given reasons for the decision, according to Hagan. The Department of Health plans to release award decisions for the various categories throughout the fall, with the final list of approved projects expected by October.

Rural health leaders across Louisiana are watching the process closely, as the funding represents a significant infusion of resources for communities that have long struggled with limited access to quality care. The outcomes of this round of awards will shape the state’s health‑care landscape for years to come.
