Families Should Recognize Co‑Occurring Mental Health Issues When Tackling Addiction

When a family member battles addiction, the focus can quickly become the substance‑use behavior itself. Yet, as AdCare Rhode Island points out, many people who struggle with addiction are also dealing with an undiagnosed or untreated mental health condition. Recognizing this link is essential for families who want to help their loved ones achieve lasting recovery.

The Overlap of Addiction and Mental Illness

Research shows that roughly half of adults in the United States with a serious mental illness also meet criteria for a substance‑use disorder. Unfortunately, treatment systems often address only one disorder, leaving the other unaddressed. Nora Volkow, director of the National Institute on Drug Abuse, emphasizes that “a critical element is overlooked: co‑occurring mental health conditions.” She also notes that in many communities it is easier to obtain illicit drugs than to access adequate mental‑health care.

Why Mental Health Issues Fuel Substance Use

Several risk factors tie the two conditions together, including trauma, adverse childhood experiences, genetic vulnerability, chronic stress, and a family history of substance misuse. Trauma, in particular, plays a major role: about 45% of people with lifetime post‑traumatic stress disorder also meet criteria for a substance‑use disorder. The relationship is bidirectional—substance use can increase exposure to traumatic events, creating a vicious cycle.

Self‑medication is another common pathway. Individuals experiencing anxiety, depression, or other psychiatric symptoms may turn to alcohol or drugs to “cope” with temporary stress or chronic mental‑health problems they may not even recognize. While this may provide short‑term relief, it typically worsens the underlying condition over time and adds new stressors that further complicate recovery.

Impact on Recovery Efforts

Untreated mental‑health issues can make early recovery especially challenging. Anxiety, mood instability, and depression can trigger cravings, increase the risk of relapse, and reduce engagement in therapy. Patients who receive treatment for only their substance‑use disorder often lack the tools to manage these psychiatric symptoms, leading many to return to drugs or alcohol quickly.

Integrated Treatment: The Best Path Forward

The National Institute on Drug Abuse and the Substance Abuse and Mental Health Services Administration both recommend an integrated approach that treats both disorders simultaneously. Integrated treatment improves quality of care, health outcomes, and the likelihood of sustained sobriety by addressing the whole person rather than isolated symptoms.

Families should look for programs that offer coordinated care—where clinicians assess and treat both mental health and substance‑use issues together. Asking providers how they integrate services, what therapies are used, and how they monitor progress can help ensure the chosen treatment plan supports lasting recovery.

Practical Steps for Families

  • Ask treatment centers about their approach to co‑occurring disorders.
  • Encourage loved ones to receive a comprehensive mental‑health evaluation.
  • Support the development of healthy coping skills, such as exercise, prayer, or community involvement, to replace substance use.
  • Stay informed about local mental‑health resources and support groups.

Just as a physician would not treat chest pain without investigating its cause, families should not expect recovery from addiction without addressing any underlying mental‑health concerns. Integrated, patient‑centered care offers the strongest chance for a stable, lasting recovery.
