Early‑life sugar limits linked to lower cancer risk later in life

Researchers analyzing data from the UK Biobank have found that limiting added sugar during the first 1,000 days of life – from conception through a child’s second birthday – may help slow biological aging and lower the risk of several major cancers up to five decades later.

Study design and natural experiment

The study, published in the Proceedings of the National Academy of Sciences, examined more than 64,000 participants born between 1951 and 1956. The investigators used a natural experiment created by post‑World War II sugar rationing in Britain, which ended in September 1953. After the rationing stopped, average daily sugar consumption in the United Kingdom nearly doubled, rising from about 41 grams to 80 grams, while intake of other major foods stayed roughly the same.

Participants who were conceived and spent their early childhood under the sugar‑rationing regime were compared with those born after the policy ended. Cancer outcomes were tracked through national health records roughly fifty years later.

Key findings

Adults who experienced more of their early lives under sugar limits showed lower rates of several cancers, including liver, rectal, lung, prostate and breast cancer. They also had longer leukocyte telomeres – the protective caps on chromosomes that shorten with age – equivalent to about two fewer years of biological aging. In addition, these individuals exhibited lower levels of Granzyme B, a protein linked to chronic inflammation and long‑term immune activation.

Possible mechanisms

The authors identified two pathways that could explain the lasting benefits. First, an early biological imprint may set a healthier baseline for metabolism and immune function. Second, early exposure to lower sugar appears to shape lasting food preferences; five decades after rationing ended, the study participants who grew up with sugar limits still reported consuming less added sugar and eating more varied, nutrient‑dense diets.

Expert commentary

“What surprised me most was how persistent the effects were,” said study author Weilong Zhang of the University of Cambridge. “This suggests that early life may be an especially important period not only for biological development, but also for establishing long‑lasting food preferences.”

Dr. Brian Slomovitz, deputy director of the Sylvester Comprehensive Cancer Center at Mount Sinai Medical Center in Miami Beach, called the research an “excellent hypothesis‑generating study,” noting that high sugar intake is associated with metabolic disruption, insulin resistance and chronic inflammation – all pathways that can contribute to cancer development.

However, Dr. Slomovitz also cautioned that the study’s natural‑experiment design has limitations. The data come from the 1950s, a period with potential biases in record‑keeping, and many cancer sub‑categories had fewer than 100 cases, making large percentage changes difficult to interpret. He also pointed out that reduced lung‑cancer risk is traditionally linked to smoking behavior rather than diet alone.

Limitations and next steps

Because the UK Biobank cohort is not fully representative of the broader British population, the findings may not apply universally. The researchers could not fully separate the impact of early biological changes from the influence of healthier adult diets maintained over a lifetime.

Zhang emphasized that the study does not advocate for eliminating sugar entirely. “Our findings support existing recommendations to limit added sugar, particularly during pregnancy and early childhood,” he said.
