Common Medications That Can Trigger Heartburn and Esophageal Irritation

Heartburn, chest discomfort, and indigestion are often blamed on diet, but many people overlook a common culprit: the medications they take every day. GoodRx, a medication‑savings platform, identified eight drug categories that can directly irritate the lining of the esophagus and stomach, leading to pill esophagitis, ulcers, or even narrowing of the esophagus.

Antibiotics – especially tetracyclines

Tetracycline antibiotics are highly acidic and can cause pain within hours of ingestion. Doxycycline, a widely used member of this class, may produce ulcers in the esophagus and stomach. Taking doxycycline with food can lessen the irritation.

Clindamycin is another antibiotic linked to esophageal damage. A small study of patients who reported painful swallowing after taking clindamycin found that all had visible esophageal ulcers on endoscopic examination.

Aspirin

Many adults take a daily low‑dose aspirin to reduce the risk of heart attacks and strokes. While aspirin inhibits platelet clumping, it also disrupts prostaglandins—molecules that protect the gut lining. This can lead to esophagitis and gastrointestinal bleeding. If you are on daily aspirin, confirm with your healthcare team that you need it and that you are using the lowest effective dose.

Nonsteroidal anti‑inflammatory drugs (NSAIDs)

Ibuprofen (Motrin, Advil) and naproxen (Aleve) belong to the NSAID family and similarly affect prostaglandins. They are a common cause of ulcers in the esophagus and stomach, which may present as heartburn, abdominal pain, or dark stools from bleeding. Patients experiencing these symptoms should discuss alternative options, such as celecoxib (Celebrex), which is generally easier on the gut.

Bisphosphonates for osteoporosis

Medications like alendronate (Fosamax) and ibandronate (Boniva) treat osteoporosis but have a long record of causing esophageal inflammation, irritation, and ulcers. In some cases, they can lead to esophageal stricture—a narrowing that makes swallowing difficult. Risedronate (Actonel) may cause fewer gastrointestinal side effects and could be a safer choice for some patients.

Potassium supplements

People with low potassium often take potassium chloride or potassium citrate. Both forms can irritate the esophagus, especially in solid tablet or capsule form. Switching to a liquid formulation may reduce heartburn for those who need supplementation.

Iron supplements (ferrous sulfate)

Ferrous sulfate, the most common iron supplement, becomes very acidic when it dissolves in saliva, potentially injuring the esophageal or stomach lining. Alternatives such as Feosol natural release or ferrous fumarate may be gentler on the gut.

Quinidine for heart rhythm disorders

Quinidine, an older drug used to treat abnormal heart rhythms like atrial fibrillation, can also damage the esophagus and cause strictures. Modern anti‑arrhythmic medications are now more commonly prescribed and may present fewer gastrointestinal risks.

Managing medication‑related heartburn

Symptoms usually improve within a few weeks after stopping the offending drug. To prevent recurrence, consider the following steps:

  • Take pills with plenty of water and remain upright for at least 30 minutes.
  • Ask your healthcare team about liquid formulations or alternative medications.
  • Use over‑the‑counter antacids cautiously, as they can interact with some prescriptions.
  • Discuss protective medications such as proton pump inhibitors (PPIs) – omeprazole (Prilosec), pantoprazole (Protonix), or esomeprazole (Nexium) – if you have frequent heartburn.

For nighttime heartburn, elevate the head of the bed or use a wedge pillow to keep acid from flowing back into the esophagus while you sleep.

Heartburn can disrupt daily life and sleep, but it is often manageable by reviewing the medications you take and working with your healthcare team to find gentler alternatives.
