Brown County residents are being encouraged to participate in a Community Health Assessment (CHA) survey to help identify the community’s most important health needs.
Community Health Assessment
The survey, which is confidential and open to anyone in Brown County aged 18 and over, will be used to inform the Community Health Improvement Plan (CHIP). This plan will outline the priorities for health improvement in the county over the next three years.
According to Chrystal Woller, Health Director/Officer for the City of De Pere, ‘A Community Health Assessment is like a doctor’s checkup for the entire community. It helps identify our most important health needs so local organizations can make informed decisions about programs, services, and resources.’
Paper copies of the survey are available at all Brown County Library locations and Brown County Public Health’s Green Bay office. The survey can also be completed online and participants can enter for a chance to win one of twenty-five $50 gift cards.
