Ankle Mobility Exercise Can Help Relieve Lower Back Pain, Experts Say

Lower‑back pain remains the leading cause of disability globally, with an estimated 619 million people suffering in 2023 and projections rising to 843 million by 2050. While many focus on the spine itself, physical‑therapy experts point to the importance of the muscles and joints farther down the leg.

Why Ankle Mobility Matters

According to Karena Wu, a doctor of physical therapy and Start TODAY trainer, proper movement of the ankle joint can improve balance and reduce tension that travels up the kinetic chain to the lower back. “Making sure the joints or the bones of the ankle are moving appropriately can actually help your balance, and it can also help with low back tightness,” Wu told TODAY.com.

Weakness or tightness in the glutes, hips, hamstrings, and ankles often contributes to lower‑back strain. By increasing ankle flexibility, the muscles that extend upward from the ankle receive better support, which can improve posture and lessen back discomfort.

The Recommended Exercise

Wu’s go‑to movement is a standing ankle‑mobility drill performed in a lunge‑like stance. Here’s how to do it:

  • Stand with your feet hip‑width apart, one foot slightly forward.
  • Keep both heels firmly on the ground.
  • Bend the front knee gently, allowing the knee to travel over the toes while the heel stays planted.
  • Aim to bring the knee about four inches in front of the toes, as Wu demonstrates with her clients.

This motion encourages the ankle joint to glide smoothly, loosening surrounding soft tissue and promoting better alignment throughout the lower body.

Benefits Over Time

Regular practice can increase ankle flexibility, which may reduce the risk of falls and provide lasting relief for lower‑back pain. As the ankle becomes more mobile, the supporting muscles work more efficiently, helping to maintain a neutral spine and encouraging healthier movement patterns throughout daily activities.

While the exercise is simple, consistency is key. Incorporating the drill into a daily routine—whether before a walk, a workout, or simply as part of a morning stretch—can yield noticeable improvements in both balance and back comfort.

Additional Tips for Back Health

Wu emphasizes that ankle mobility should complement, not replace, a comprehensive approach to back health. Strengthening the glutes, hips, and hamstrings, maintaining good posture, and staying active are all essential components of a pain‑free lifestyle.

For anyone experiencing persistent lower‑back pain, consulting a qualified physical therapist can provide personalized guidance and ensure that exercises are performed safely and effectively.
