Study Finds Common Osteoporosis Drugs May Lower Alzheimer’s Risk

Researchers in Hong Kong have reported a striking association between widely used osteoporosis medications and a lower likelihood of developing Alzheimer’s disease or other dementias. The study, published in the journal Alzheimer’s & Dementia, examined 121,492 adults aged 60 and older who had recently been diagnosed with osteoporosis or suffered a serious fracture involving the spine, upper arm, wrist, or hip.

Key Findings

Participants who were prescribed nitrogen‑containing bisphosphonates (NBPs)—including alendronate, ibandronate, risedronate and zoledronate—experienced a 16% lower risk of Alzheimer’s disease or a related dementia compared with those who received no osteoporosis medication. When compared with patients taking other types of osteoporosis drugs, the risk reduction was even larger, at 24%.

What the Researchers Say

Ching‑Lung Cheung, an associate professor of pharmacology at the University of Hong Kong’s Li Ka Shing Faculty of Medicine and a co‑author of the study, described the results as evidence of a possible “bone‑brain axis.” He noted that prior research has shown people with low bone mass are more likely to develop dementia later in life, underscoring the importance of maintaining strong bones as we age.

“Our findings therefore do not prove that bisphosphonate use reduces dementia risk,” Cheung told Fox News Digital. He emphasized that the study is observational, meaning it can identify associations but cannot establish cause and effect. He called for clinical trials to confirm whether the drugs truly protect brain health.

Limitations and Cautions

The authors highlighted several limitations. The study relied on existing health records and did not include baseline cognitive testing, leaving open the possibility that some participants already had early cognitive impairment. Information on the severity of osteoporosis, fracture details, and use of over‑the‑counter calcium or vitamin D supplements was also missing.

Other factors that influence dementia risk—such as education level, social isolation, air quality, and vision loss—were not fully accounted for in the analysis. Because the research focused exclusively on a Chinese population, it remains unclear whether the results apply to other ethnic groups.

Implications for Patients and Doctors

While the findings are encouraging, Cheung cautioned that they do not justify prescribing bisphosphonates solely to prevent Alzheimer’s disease. The primary purpose of these medications remains the treatment and prevention of bone loss and fractures.

He suggested that, if future clinical trials confirm a protective effect, doctors might have an additional tool for managing dementia risk, especially for older adults already taking these drugs for bone health.

Next Steps

The research team plans to pursue randomized clinical trials that can more definitively test the link between bisphosphonate therapy and brain health. They also hope to explore whether sex or other demographic factors modify the observed association.

In the meantime, the study adds to a growing body of evidence that bone health and brain health may be interconnected. Maintaining strong bones through proper nutrition, weight‑bearing exercise, and appropriate medication continues to be a cornerstone of healthy aging.
