Federal investigators examine Amazon cargo jet runway overrun that killed five in Miami

Miami International Airport became the focus of a federal investigation after an Amazon cargo jet overran the runway on Sunday, killing five people and injuring several others. The National Transportation Safety Board (NTSB) dispatched a team led by Chairwoman Jennifer Homendy to determine whether the aircraft touched down too late, exceeded safe speed limits, or suffered mechanical issues.

Key points of the investigation

Investigators will analyze the exact touchdown point, aircraft speed, braking performance, weather conditions, and the plane’s mechanical health. Video footage suggests the Boeing 767 remained airborne well beyond the normal landing zone, which is typically within the first 3,000 feet of a runway. Aviation safety expert Steve Arroyo notes that if the jet landed beyond that zone, pilots should have aborted the approach and attempted another landing.

Former Transportation Department Inspector General Mary Schiavo, who is also a pilot, observed that the aircraft appeared to “float” above the runway without the usual nose‑up flare that helps settle the plane onto the landing gear. She added that strong tailwinds reported at the time could have contributed to the extended float.

Airport safety measures

Miami International does not have the FAA‑approved arresting systems installed at more than 120 U.S. airports, which use crushable material at runway ends to stop overrunning aircraft. The airport complies with FAA requirements because it maintains a 1,000‑foot safety buffer beyond the runway, according to former FAA airport‑safety director Mike O’Donnell.

The crash halted operations at the busy hub for much of the afternoon, leading to the cancellation of over 160 flights and delays to nearly 325 others. Some flights were diverted to Orlando International Airport, more than 200 miles north.

Amazon’s response

Amazon issued a statement expressing deep sorrow for the loss of life, offering condolences to the families of the victims and pledging full cooperation with the investigation.

The cargo jet, operated by 21 Air, a North Carolina‑based carrier, was a 32‑year‑old Boeing 767 originally built for passenger service and converted to a freighter in 2015. Schiavo noted the aircraft’s maintenance history, including prior foreign ownership, will also be scrutinized.

Historical context

Runway overruns have prompted the FAA to recommend arresting systems after incidents such as the 1999 American Airlines Flight 1420 crash in Little Rock, which claimed nine lives. The agency credits these systems with saving hundreds of lives nationwide.

Last year, a UPS cargo plane suffered a runway accident in Louisville, Kentucky, killing three pilots and 12 people on the ground, underscoring the ongoing safety challenges in cargo aviation.
