Common Medications That Can Trigger Heartburn and How to Manage Them

Many people experience heartburn, chest pain, or indigestion without realizing that a medication they take could be the culprit. GoodRx, a medication‑savings platform, identified eight common drugs that can irritate the lining of the esophagus and stomach, sometimes leading to pill esophagitis or ulcers.

Antibiotics – especially tetracyclines

Tetracycline antibiotics are highly acidic and can cause immediate pain after a dose. Doxycycline, a widely used tetracycline, is known to cause esophageal ulcers. Taking doxycycline with food can help reduce irritation. Clindamycin is another antibiotic linked to esophageal erosions; a small study found that every participant who reported pain while swallowing after clindamycin use had visible ulcers on endoscopy.

Aspirin

Daily low‑dose aspirin is often prescribed to prevent blood clots that lead to strokes and heart attacks. However, aspirin interferes with prostaglandins, the molecules that protect the gut lining, increasing the risk of esophagitis and gastrointestinal bleeding. If you take aspirin, confirm with your healthcare team that you need it and that you are on the lowest effective dose.

Nonsteroidal anti‑inflammatory drugs (NSAIDs)

Ibuprofen (Motrin, Advil) and naproxen (Aleve) also reduce prostaglandin production and are common causes of ulcers in the esophagus and stomach. Symptoms may include heartburn, stomach pain, or dark stools from bleeding. Switching to a different NSAID such as celecoxib (Celebrex), which is gentler on the gut, can be an option after consulting your provider.

Bisphosphonates for osteoporosis

Medications like alendronate (Fosamax) and ibandronate (Boniva) treat osteoporosis but have a long history of causing esophageal inflammation, irritation, and ulcers. They can also lead to narrowing of the esophagus (esophageal stricture). Risedronate (Actonel) may cause fewer gastrointestinal side effects and could be a safer alternative.

Potassium supplements

People with low potassium often take potassium chloride or potassium citrate. Both forms can irritate the esophagus, especially in solid capsule or tablet form. Switching to a liquid formulation may lessen discomfort.

Iron supplements

Ferrous sulfate, the most common iron supplement, becomes very acidic when it dissolves in saliva, potentially injuring the esophageal lining. Alternatives such as Feosol natural release or ferrous fumarate formulations are generally easier on the gut.

Quinidine for heart rhythm disorders

Quinidine, used to treat abnormal heart rhythms like atrial fibrillation, can also damage the esophagus and cause strictures. Modern heart‑rhythm medications are often preferred and may present fewer gastrointestinal risks.

Managing heartburn caused by medication

Symptoms usually improve within a few weeks after stopping the offending drug. To protect the esophagus while you remain on necessary medication, consider the following steps:

  • Take pills with plenty of water and remain upright for at least 30 minutes.
  • Ask your healthcare team about liquid or chewable formulations when available.
  • Discuss protective medications such as proton pump inhibitors (PPIs) – omeprazole (Prilosec), pantoprazole (Protonix), or esomeprazole (Nexium) – which reduce stomach acid and promote healing.
  • For occasional heartburn, antacids or H2 blockers may be sufficient; for frequent or chronic symptoms, PPIs are often the safest long‑term choice.

Practical tips for nighttime heartburn

If heartburn disrupts sleep, elevate the head of the bed or use a wedge pillow to keep the upper body raised. Avoid lying flat after meals and steer clear of trigger foods such as spicy or fatty meals.

Heartburn can interfere with daily life, sleep, and overall well‑being. If you suspect a medication is the cause, contact your healthcare team promptly. There are often alternative treatments or protective strategies that can keep your digestive tract healthy while you continue to manage your underlying health conditions.
