Ionis and Novartis report late‑stage trial failure of heart‑drug pelacarsen

Ionis Pharmaceuticals and Swiss drugmaker Novartis announced on Friday that their experimental heart medication pelacarsen failed to meet its primary endpoint in a late‑stage clinical trial. The study, which enrolled patients with elevated levels of the cholesterol‑carrying particle lipoprotein(a), showed that the drug did not significantly lower the risk of major cardiovascular events such as heart‑related death, heart attack, stroke, or urgent procedures to restore blood flow.

Trial results and investor reaction

While pelacarsen continued to demonstrate a robust reduction in lipoprotein(a) concentrations—consistent with earlier Phase II data—the anticipated translation into real‑world cardiovascular risk reduction was not observed. In the wake of the announcement, Ionis shares fell about 6 % in aftermarket trading.

Implications for the companies

Pelacarsen is one of three high‑profile experimental products that Novartis is counting on for future growth, alongside the multiple‑sclerosis candidate remibrutinib and the gene‑therapy del‑desiran. Together, these assets represent more than $10 billion in projected peak annual sales, a key component of Novartis’s long‑term strategy.

Ionis CEO Brett Monia expressed disappointment, stating, “Although pelacarsen substantially lowered lipoprotein(a) levels consistent with previous studies, we are disappointed that this did not translate to cardiovascular risk reduction.” The company indicated it will continue to evaluate the data and explore alternative pathways for addressing elevated lipoprotein(a) in patients.

Context within cardiovascular research

Elevated lipoprotein(a) has been identified as an independent risk factor for atherosclerotic disease, prompting significant interest from the pharmaceutical industry. The failure of pelacarsen to demonstrate a clear clinical benefit underscores the challenges of turning biomarker reductions into tangible health outcomes.

Analysts note that the setback may delay the timeline for bringing a novel lipoprotein(a)‑targeting therapy to market, but they also point out that the broader pipeline of cardiovascular drugs remains active, with several other companies pursuing alternative mechanisms.

Looking ahead

Both Ionis and Novartis said they remain committed to advancing innovative treatments for heart disease and will assess next steps based on the trial’s full data set. Investors and patients alike will be watching for any further announcements regarding additional studies or potential adjustments to the development program.
