Brazilian air accident investigators have released a report on the 2024 crash of Voepass Flight 2283, which killed all 62 people on board. The report cites a buildup of ice, failures in the aircraft’s de-icing system, and crew actions as contributing factors to the crash.
Investigation Findings
The investigation found that the aircraft encountered conditions favorable to severe icing, and the plane’s de-icing system experienced multiple failures. The pilots switched the system on and off several times amid icing warnings. A total of 19 factors contributed to the crash, including the crew’s emotional state, external influences, adverse weather conditions, and insufficient regulatory oversight.
The report also pointed to broader shortcomings at Voepass, describing a ‘normalization of deviations’ within the airline. This included a culture of informality in which pilots and mechanics did not record aircraft-related issues in maintenance logs, allowing the aircraft to continue operating despite known issues.
